Details:

• For each person that you refer to our practice and begin treatment by Sept 1, 2011 your name will be entered into our IPAD contest.
• In addition as an extra incentive for every referral that begins treatment with us you both will receive a $50.00 credit toward your account once treatment begins.
• Enter by email to ortho@affds.com  or in person.  We will track their start date and enter your name to the drawing.
• Contest winner will be contacted by phone and posted to our website by September 10th.
• We will also send you an email confirming that you have been entered and who started treatment with us.
